A reminder!

All your updated CIP forms are to be handed in to me latest by this friday! Preferably Thursday. All changes are to be made in RED and you'll need letters or certs of verification for the events that you added in!

For missing activity:
-Check if CIP activity is in the list of activities(06-07) in the IVLE. If present. indicate serial no(e.g event#2), name of activity and no of hours on cert.
-If not found, refer to list of activities with missing information. if activity is there, write down the missing info on ur CIP cert.
- Attach letter of evidence
- Endorsement letter should include: Full name, class, NRIC, name of organizer, brief description, date of start of activity. hours rendered and signature of person in charge/stamp
-If you have activities organized by CCA groups or school, look for the teacher in charge to endorse

Yepp.. all these to be done by FRIDAY! and hand it in to me!

Next, the personal qualities survey form, please complete and write for your friend and hand in by FRIDAY to QIWEN!!

AND. hand in the copy of ur CCA testimonial given back to QIWEN by FRIDAY also. And make only minor changes if necessary.

OK! that should be all.. remember to do the Bio survey online!!!
